Delete Your Account & Data
You can request deletion of your CareSheet account and the personal data associated with it using the form below. This page is also usable if you no longer have access to the app or your login.
What happens when you submit this request
- We verify your identity using the details you provide, then permanently delete your account credentials, profile information (name, email address, phone number), and personal data associated with your account.
- Requests are processed within 7 working days, and you will receive a confirmation at the email address you provide once deletion is complete.
- This action is permanent and cannot be undone. Data that is deleted cannot be recovered.
- Certain records may be retained where retention is required by applicable law, regulation, or legitimate medical record-keeping obligations. Any retained data is minimized and kept protected in accordance with our Privacy Policy.
- Deleting your account does not automatically delete records that a clinic maintains about you as a patient. Those records are governed by the treating clinic's retention policy; your request will be forwarded to the relevant clinic administrator where applicable.
Account deletion does not cancel store subscriptions. Manage renewal through Apple subscriptions or Google Play subscriptions. You can still submit your deletion request below.
Other ways to delete your account
- Mobile app: Go to More → Account → Delete Account to request deletion directly from the iOS or Android app.
- Email: Write to [email protected] with the subject "Account Deletion Request" from your registered email address.
